The first three columns of data contain the response data, and the 4th to the 8th contain the codes for the covariates. The codes for the covariates are 1,2,3..., as applicable, and the order of the codes corresponds to the order given in Tables 1 and 2. For example for the first covariate (agegroup)in column 4 the codes are 1= under 19 2= 19-20 3= 21-22 4= 23-24 5= over 24 The file layout is as follows, with all numbers right justified in the fields.
