Computer modelisation of the prothrombin time test. Conception, validation and potential of application

Computer simulations are more and more widely used in the understanding of complex biological systems. As a first attempt to study their usefulness in the field of blood coagulation, we developed a computer model of the prothrombin time (PT) test. Biochemical interactions between factors II, V, VII, X and fibrinogen were modelled according to a detailed analysis of the literature, leading to a system of ordinary differential equations. PT's and coagulation factors (I, II, V, VII and X) were measured in 50 patients; these concentrations were set in the computer model and PT simulations were run. Comparison of in silico and in vitro PT's gives a Pearson correlation coefficient of 0.90, stressing the relevance of the simulations. International Normalised Ratio's (INR), PT's and coagulation factors were then measured in 10 patients taking oral anticoagulants. Correlation between in silico and in vitro PT's and INR's show both a Pearson coefficient of 0.98, strongly supporting the accuracy of the model in this context. We therefore expose how it could be useful in predicting the response of a patient when initiating an oral-based anticoagulant treatment. Moreover, we describe how it can predict the ideal dose of Kaskadyl© or Novoseven© to administer to patients with overdose of oral anticoagulants in order to decrease their INR to a safe value.
