Abstract
Introduction: Diagnostic management that does not adhere to guidelines is frequent and potentially harmful in patients suspected of pulmonary embolism (PE). We evaluated the effectiveness of a computerized decision support system on Personal Data Assistant (PDA) aimed at improving the quality of diagnostic practices in emergency departments (ED).
Methods: A cluster multicenter randomized control trial was conducted in 20 EDs. For each patient who underwent diagnostic testing for suspected PE, physicians completed a standardized form on PDA. After a period of baseline evaluation, EDs were randomized in passive dissemination of written guidelines or decision support system implemented on the PDA. We measured the appropriateness of diagnostic management according to international guidelines (main criterion) and the incidence of thromboembolic events during 3-month follow-up.
