Abstract

Introduction: Coronary artery disease (CAD) patients on dual antiplatelet therapy with a residual platelet reactivity (RPR) may be at increased risk of ischemic vascular events. VerifyNow P2Y12 assay, a new point-of-care (POC) test, has been suggested as rapid tool to evaluate RPR in CAD patients on clopidogrel

Methods: We compared this POC system with light transmission aggregation (LTA) induced by 2 and 10 mM L ADP in 1267 CAD patients undergoing percutaneous coronary intervention and receiving dual antiplatelet therapy. Cut-off values for identifying RPR were defined on the basis of 5th percentile of normal distribution: 52% and 64% for 2 and 10 mM L ADP LTA and 244 Platelet Reaction Unit (PRU) for VerifyNow system

Results: RPR was detected in 16.9% patients by 2 mM ADP-LTA, in 29.2% by 10 mM ADP-LTA and in 30.5% by VerifyNow method. VerifyNow P2Y12 assay showed a significant correlation with LTA induced by 2 mM ADP (rho=0.62, p<0.001; concordant samples =74.5%, k=0.33, p<0.001) and with LTA induced by 10 mM (rho=0.63, p<0.001; concordant samples=75.7%, k=0.43, p<0.001). Considering 10 mmol/L ADP-LTA as the reference method for RPR, VerifyNow system showed a sensitivity of 63.3%, a specificity of 83.3%.

Conclusions: Our results show good significant correlations between POC system and LTA. Clinical validation studies for POC system are necessary
