Abstract

Introduction: Hypercoagulopathy is one of the new defined complications of thalassemia, and high incidence of right side heart failure in some researches (due to pulmonary hypertension), is indicated on the importance of this complication.

In this research we evaluate the frequency of protein C, S, and antithrombinIII (AT-III) deficiency in thalassemia major patients in Isfahan.

Methods: Serum level of protein C, S and AT-III were measured in 41 thalassemia major patients (age 7-20 years); and if any deficiencies, relation to age, sex, splenectomy, transfusion interval, pre transfusion Hb, and serum ferritin level. All of these patients were also evaluated for cardiac disease, and pulmonary hypertension with doppler echocardiography.

Results: 43% had protein C deficiency, 53% protein S deficiency, and 30% AT-III deficiency. 77.5% of all patients had at least one factor deficiency. The frequency of protein C, S and AT-III deficiency was higher in patient older than 10 years, with transfusion interval more than 21 days, and also less pre-transfusion Hb level. High ferritin level was also significantly related to protein C and S deficiency (but not AT-III) (P<0.05).

With echocardiography, dilated heart were found in 8 (11.8% in non splenectomised, and 57% in splenectomised) (PV<0.001), but only 2 patients with some evidence of pulmonary hypertension.

Conclusions: Also hypercoaglopaty due to protein S, C and AT-III deficiency were common (especially with inadequate transfusion and chelating therapy), but unlike some researches, we didn't find high incidence of pulmonary hypertension in our patients.
