Abstract
Introduction: TEL- AML1 t(12;21)(p13:q22) is the most frequent genetic rearrangement in acute lymphoblastic leukemia.
Methods: Fifty five cases with ALL(including 10 relapsing ones) were included in this study. They received induction, intensification and maintenance chemotherapy. Also, they received cranial irradiation to a total dose of 1800 cGY in 2 weeks (10 fractions). Again, intrathecal chemotherapy was given. Determination of MRD Was done using real time PCR to detect t(12:21) (p13: q22) TEL-AML1.
Results: It was found that TEL-AML1 was positive in 36.79% of the newly diagnosed cases with a mean ratio of 0.560.29 while in relapsed patients, it was positive in 33.3% of cases with a mean ratio of 0.850.16. Again, it was found that patients with good response to chemotherapy showed a lower TEL-AML1 ratio (0.02-0.28) while those with slow or bad response to chemotherapy showed higher ratios(0.420.1 and 0.760.14 respectively). A positive correlation was found between TEL-AML1 ratio and hepatomegaly, splemegaly, lymphadenopthy and FAB stage classification. On the other hand, no correlation between TEL-AML1 ratio and other hematological parameters e.g.haemoglobin, total leucocyric coun, platelet count and blast cell mass.
Conclusions: Quantification of TEL-AML1 may play a role in predicting the response to chemotherapy in cases with ALL.
