Abstract
Introduction: Non-Arteritic Ischemic Optic Neuropathy (N-AION)is caused by acute ischemic infraction of the optic nerve lead,which is supplied by the posterior cilliary arteries.
We already have reported that increased fibrinogen values and the prothrombin G20210A polymorphism are common findings between N-AION patients.
In this study we investigate whether there is an association between N-AION and plasma protein z levels.
Methods: Nineteen cases of confirmed N-AION and thirty-one healthy controls of the same age consist the study groups.Protein z was estimated in thawed citrate plasma on cases and controls by a commercial enzyme immunoassay(Asserachrom,Protein Z)in one experiment,the same day by the same batch of reagents.Results are calculated from a standard curve and expressed in ng per ml of plasma.The imprecission of the estimation calculated from the values of a normal plasma was satisfactory(CV=4.6%).
Results: The control's protein z values distributed within a range from 340 to 2000 ng/ml(median=1040,mean=1166,SD=443 ng/ml).Accordingly,the patient's protein z values distrited within a range from 420 to 2200 ng/ml(median=1020,mean=1079,SD=408).There is not any statistical difference between the two distributions (Mann-Whitney U test,P=0.41) but the distribution of the patient's values was occupying approximately the first three quartiles of the distibution of the values of controls.There is only one N-AION case with a value of protein Z bigger than 1500 ng/ml(1/19, 5% of cases).In contrast,six controls had a value of protein Z bigger than 1500 ng/ml(6/31, 19% of controls).
Conclusions: High values of protein z in plasma, especially values bigger than the normal values of the last quartile are not observed in N-AION patients.This new finding can be used mainly as a negative diagnostic criterion,along with the increased fibrinogen level and the high frequency of G20210A polymorphism of prothrombin gene, in future studies on the pathophysiology of N-AION.
