ACUTE EFFECTS OF LIDOCAINE ON NERVE GROWTH FACTOR (NGF)-EVOKED HYPERALGESIA IN HUMAN

Objective: NGF injection into human skin causes thermal sensitization and sustained mechanical hyperalgesia. We explored this model for microdosing using local injections of lidocaine. Methods: NGF (1 mg/50ml) was injected intradermally into human (n=12) lower leg. One and 3 weeks later, sensitization to heat and mechanical impact stimuli was investigated prior to and 5 min after injection of lidocaine (150ml of 0.01% and 0.1%) at the hyperalgesic NGF and control skin sites. Results: NGF caused heat and mechanical hyperalgesia at week 1 and 3. Local lidocaine (0.1%) reduced heat hyperalgesia at the NGF site (week 1, p < 0.004). Unexpectedly, heat thresholds were sensitized by lidocaine at the control sites and 3 weeks after NGF injection (p < 0.05). Mechanical impact pain ratings were reduced by lidocaine dose-dependently in control and in NGF-treated skin, both at 1 and 3 weeks after injection. Conclusions: After NGF administration, local lidocaine injections can reduce suprathreshold mechanically-induced pain, whereas heat pain thresholds might be sensitized. While sodium channel interaction can explain the analgesic effects, activation of TRPV1 by lidocaine may be linked to sensitization to heat. We conclude that microdosing in the human NGF model can be performed with local anaesthetics as translational model for neuropathic hyperalgesia.
